Transitioning from High School
Jeffco Transition Services School programs can help students with disabilities bridge the gap from school to postsecondary activities and life, preparing them for adulthood and providing them with skills they will need to obtain gainful employment, if appropriate.
Transition IEP
When a student turns 15 or enters ninth grade, additional sections appear in their Individualized Education Plan document regarding transition. A transition assessment will be completed to inform postsecondary goals and course of study.
Transition services and activities may include programming such as the School to Work Alliance Program (SWAP), career exploration and assistance from outside agencies.
