Hub
There are several Jeffco Transition Services Hub programs located across Jeffco Public Schools. Hub programs have a greater level of support than JTS community programs. Hub programs provide instruction that develops and strengthens students’ academic abilities, vocational skills, adult living skills, and others.

Recommended Agencies
Division of Vocational Rehabilitation (DVR)
DVR services are available to young adults choosing to participate beginning at age 15. Services at this time focus on developing important employment skills that will lay a foundation for success after leaving school.
Developmental Disability Resource Center (DDRC)
DDRC’s mission is to create opportunities for people with intellectual and developmental disabilities and their families to participate fully in the community.
RTD Access-a-Ride
Transition students rely on public transportation. Access-a-ride offers individuals with disabilities free access to transportation. Access-a-Ride is RTD’s ADA complementary paratransit service. It is a shared-ride service intended for those who cannot use regular fixed-route bus/light rail service due to a disability.
Supplemental Security Income (SSI)
SSI provides monthly payments to people with disabilities and older adults who have little or no income or resources.
Medicaid
Health First Colorado is public health insurance for Coloradans who qualify. Medicaid is funded jointly by the federal government and Colorado state government and is administered by the Department of Health Care Policy and Financing. Each state manages its own Medicaid program differently.
ARC Advocacy
Advocacy helps individuals and families find information and resources, navigate complex systems, solve problems, and overcome obstacles.
